You can fill out all your necessary paperwork ahead of time with our on-line registration (see insurance link, "patient forms") or, if you prefer, our receptionists will be happy to assist you at the time of your initial visit.

CONFIRMATION OF INSURANCE BENEFITS PRIOR TO RECEIVING CARE
At Apex we feel strongly all patients should be informed of any financial responsibilities up front. Our receptionist will make every attempt to verify and confirm with you your benefits and any financial obligations prior to starting therapy.


CONVENIENT LOCATIONS WITH AMPLE PARKING
Both the San Mateo and San Carlos clinics are conveniently located immediately off the 101 freeway. There is ample parking at both clinics with handicapped parking available.


STATE OF THE ART EQUIPMENT WITH AMENITIES FOR YOUR CONVENIENCE
We hope you immediately notice that Apex spared no expense in furnishing our clinics with state of the art rehabilitation equipment. Please feel free to use our changing rooms which include necessary amenities such as deodorants, hair products, body lotions and the like.
