APEX ACCEPTS PPO'S, MEDICARE AND WORKERS COMPENSATION INSURANCES

In California, all insurance companies require that you have a prescription from a physician to be reimbursed for physical therapy. This prescription usually comes from an orthopedic surgeon but can be obtained from any doctor such as your primary care physician or pediatrician.

If you choose not to see a physician, but do have a diagnosis, we can treat you at a cash rate. If you choose this path we may request, based on your symptoms, that you see a physician to rule out more severe or unusual causes of joint pain.

HMO’s
Unfortunattely, Apex is not contracted with any HMO’s. Unless you are pre-approved, we would ask that if you have an HMO medical plan and wish to be seen at Apex that you pay a cash rate. Should we receive compensation from your HMO we will gladly re-imburse you.

PPO’s
Between the two clinic locations, Apex is in network with most major insurance companies including Blue Shield, Cigna, Health Net and Great West. If were not in-network with your particular carrier, in almost all cases we can work with you and your plan.

MEDICARE
Each of our therapists are approved Medicare providers.

WORKERS COMPENSATION
Apex accepts worker’s compensation injuries. In addition to having a referral from your physician, it will greatly expedite your care if you can provide us with as much information as possible regarding your workers compensation carrier, your case manager, and any previous therapy that you have had to date on this injury.
